Durable Power of Attorney for Health Care Decisions - NV [7 pages, MS Word].
This is the form recognized by all health care facilities in Nevada. It is sometimes referred to as a Living Will. The Nevada Secretary of State has a "lockbox" program where, you can upload this form, once fully executed, and a participating health care facility can access it if you are receiving medical assistance, or during an emergency.
